What is your financial policy?

Payment is expected at the time of service unless other arrangements have been made. Payment may be made by cash, check, money order, VISA, Mastercard, American Express and Discover.

How do I get a refill on my prescription?

Whenever possible, prescription renewals should be called in two days before your supply is exhausted. Please have the telephone number of your pharmacy available. Prescription refills must be called in during working hours.

What are your office hours?

We see patients in our New Haven office on Monday through Saturday. Our New Haven office hours are 8:00 a.m. to 6:00 p.m. M-F.

What paperwork should I bring?

Be sure to bring your insurance card, referral if required by your insurer, and workers compensation authorization, if needed.
